Testing Services

Testing Services Process

Proctored testing requests will be handled by the Office of Accessibility. Three types of requests will be handled:

  1. Accommodation testing with parameters set by a valid Letter of Accommodation from the Office of Accessibility.
  2. By permission of instructor. This may be extended to any student who needs to make up a test and is allowed to do so by their instructor. 
  3. University of North Dakota testing (Engineering).

General Procedure

  • In order to request testing, students should fill out the form on this page: https://sites.sienaheights.edu/accessibility/testing-services/
  • The information from this form will be transmitted to testingcenter@sienaheights.edu.
  • Requests should be handled by the Director of Accessibility or the Senior Director of Student Success. 
  • After verification by the faculty member (see below), a time should be coordinated with the student and the student should be given information about where to go to check in and take the test.
  • The testing time and place should be logged on the Outlook calendar associated with testingcenter@sienaheights.edu. 

By Permission of Instructor

  1. After the request is received in the inbox, the Director of Accessibility or the Senior Director of Student Success should reach out to the instructor with a testing services cover sheet (see additional attachment) asking them to verify the student request. The completed cover sheet and all test materials should be returned by the instructor. Materials can be submitted in hard copy or digitally.
  2. Coordinate with the student on the day and time for the test and let the student know the location.
  3. Log the test time and place in the calendar. Include the course number in the calendar entry.
  4. When the student arrives to take the test, check their ID.
  5. Ensure that the appropriate conditions are met. In most cases, students should leave cell phones and backpacks in a separate location from the testing location, for example.
  6. Once the test is completed and collected, notify the instructor based on the pick up option indicated on the cover sheet.

Testing for students with approved Letter of Accommodation

  1. After the request is received in the inbox, the appropriate administrative assistant should contact the instructor to verify the student’s accommodation conditions via their submitted Letter of Accommodation. If the student does not have accommodations on file, they should be directed to Michael Orlando for more information.
    1. Note: it is important to maintain the confidentiality of student accommodations; only the specific accommodation needs related to testing will be confirmed.
  2. Reach out to the instructor asking them to complete the cover sheet. All test materials should be returned by the instructor. Materials can be submitted in hard copy or digitally.
  3. Coordinate with the student on the day and time for the test and let the student know the location. Be sure that the location will meet the student accommodation needs.
  4. Log the test time and place in the calendar. Include the course number in the calendar entry.
  5. When the student arrives to take the test, check their ID.
  6. Ensure that the appropriate conditions are met and are in accordance with the student’s approved accommodations. In most cases, students should leave cell phones and backpacks in a separate location from the testing location, for example.
  7. Once the test is completed and collected, notify the instructor based on the pick up option indicated on the cover sheet.

University of North Dakota testing (Engineering)

Testing for students in an Engineering program through the University of North Dakota is facilitated through ProctorU. The STEM Division will coordinate these requests. The testing office is in the Library and can be booked for these testing needs. Follow the instructions in “By Permission of Instructor” above. Indicate “UND” as part of the calendar entry when logging these appointments.

Testing Services: Online Submission Form

To submit an exam online

  • Fill out the form below. All required fields have an asterisk and must be completed in order to submit an exam
  • Attach the exam when prompted; you also may attach one additional document
  • Click on the “Submit” button to complete the submission of the exam and testing form
  • Students are responsible for scheduling an appointment to take a test. Tests must be scheduled at least one week in advance to allow for staffing.
  • Online accommodation testing submissions will have a processing time of 1-2 business days.
